We’ve streamlined the process to submit an update to the account holder name if you’ve legally changed your name.
How to submit your request
- Access the form: on this page.
- Prepare your documents: You’ll need to provide a digital copy of one of the following documents:
- Name Change Certificate
- Birth Certificate
- Driver's License
- Passport
- Note: Please ensure that the documents are clear and legible. Documents are not required if your name change is due to marriage or divorce.
- Complete the form: Fill out the form on our website. You’ll need to include your account number to ensure that your request is processed efficiently.
- Note: you can find your account number in the top left corner of your PDF bill.
- Submit your request: Once you’ve filled out the form and attached all required documents, submit your request through the website.
If you have a Business, Corporate or Government account, you must fax or mail these documents.
