Account holder name change request

We’ve streamlined the process to submit an update to the account holder name if you’ve legally changed your name.

How to submit your request

  • Access the form: on this page.
  • Prepare your documents: You’ll need to provide a digital copy of one of the following documents:
  • Name Change Certificate
  • Birth Certificate
  • Driver's License
  • Passport
  • Note: Please ensure that the documents are clear and legible. Documents are not required if your name change is due to marriage or divorce.
  • Complete the form: Fill out the form on our website. You’ll need to include your account number to ensure that your request is processed efficiently.
  • Note: you can find your account number in the top left corner of your PDF bill.
  • Submit your request: Once you’ve filled out the form and attached all required documents, submit your request through the website.


If you have a Business, Corporate or Government account, you must fax or mail these documents.
