Ontario Works (OW)
To apply, you must:
- Upload or attach your most recent Old Age Security T4A (OAS) statement. To protect your privacy, remove your Social Insurance Number (SIN) or Old Age Security (OAS) number from your documents before uploading. Please ensure that your name, address and contents of Box 21 are clearly visible.
- Live at an address that has Rogers service (go to rogers.com, select a product then click Check Eligibility to check your address).
Note: You must upload or attach your most recent OW statement with your name and address clearly visible.

Be sure to include the PDF printed copy of the application form along with all necessary documentation.
If a third party is applying on your behalf, include this Letter of Authorization with your application. We’ll process your application within 4 weeks of receiving it.

I’ve submitted my application. What happens next?
- We’ll review your documentation to confirm your eligibility into the program. Then, delete or shred them for your protection.
- Once approved, you’ll receive a call and email from a dedicated Connected for Success representative to set up your account. You’ll need to provide two pieces of Government issued ID (e.g. Social Insurance Number (SIN), Drivers License, Passport or Provincial photo ID.
